Your name on the form
In Ecuador you carry two surnames. On the USCIS form there is one single box for both.
Your Ecuadorian passport carries them in an order. That order is copied identically on every form.
The Registro Civil cédula carries them too. If they differ, use the passport name and note the other one.
Write your name the same way on every form. A surname that changes from page to page raises questions.
The “State or Province” box
USCIS asks for a “state or province”. In your country that level is called a province.
In Ecuador, “State or Province” means your province: Pichincha, Guayas, Azuay, Manabí.
Quito is in Pichincha and Guayaquil is in Guayas. City and province almost never share a name.
Cantones and parroquias belong in the address. They are not provinces.
USCIS mails you at the address you write down. Check it before you sign.
The ñ, the accents and your passport
Your passport reads “BOLAÑOS” at the top and “BOLANOS” in the machine lines. The box takes the top one.

Your national ID is not your A-Number
The Registro Civil cédula is Ecuadorian. It is not your A-Number nor a number USCIS issues.
The A-Number comes from USCIS, not from your country. It appears on the papers USCIS has sent you.
