Your name on the form
In Spain you carry two surnames and both are legal. The USCIS surname box is a single one.
The order of the surnames may be chosen when a child is registered. Copy the order your passport shows.
Hyphenated compound surnames are a single surname. Do not split them across two boxes.
Write your name the same way on every form. A surname that changes from page to page raises questions.
The “State or Province” box
USCIS asks for a “state or province”. In your country that level is called a province.
In Spain, “State or Province” is usually your province: Madrid, Sevilla, A Coruña, Alicante.
The autonomous community is a different level. Andalucía is not a province; Sevilla and Málaga are.
Ceuta and Melilla are autonomous cities, not provinces. Even so, their name is what goes in that box.
USCIS mails you at the address you write down. Check it before you sign.
The ñ, the accents and your passport
Your passport reads “PEÑA” at the top and “PENA” in the machine lines. The box copies the top one.
We print on your form exactly what you type. Accents do not disappear on their own.
Your national ID is not your A-Number
The NIE is Spain's foreigner number, not the USCIS one. Your A-Number comes from USCIS.
The A-Number comes from USCIS, not from your country. It appears on the papers USCIS has sent you.

We prepare your form, you print it, sign it by hand and mail it. Being abroad does not change that.
We publish no consulate address, phone number, hours or appointment rule. The State Department handles those.
