Your name on the form
In Honduras you use two surnames and USCIS has one box. Neither is lost: both go in together.
Your Honduran passport governs the order. Copy it the same way on every form you fill in.
Your birth record and your passport should say the same thing. If not, note the other name in its section.
Write your name the same way on every form. A surname that changes from page to page raises questions.
The “State or Province” box
USCIS asks for a “state or province”. In your country that level is called a department.
In Honduras, “State or Province” means your department: Cortés, Yoro, Olancho, Francisco Morazán.
San Pedro Sula is a city in the department of Cortés. The city and the department are not named alike.
Colonias and barrios belong on the address line, never in the department box.
USCIS mails you at the address you write down. Check it before you sign.
The ñ, the accents and your passport
Your passport reads “CÓRDOVA” at the top and “CORDOVA” in the machine lines. The box takes the top one.
We print on your form exactly what you type. Accents do not disappear on their own.
Your national ID is not your A-Number
The Honduran DNI is issued by the RNP. It is not your A-Number nor your USCIS receipt number.
The A-Number comes from USCIS, not from your country. It appears on the papers USCIS has sent you.

We publish no consulate address, phone number, hours or appointment rule. The State Department handles those.
