Copy of every page of your passport
Where to find it: Scan or photograph every page, including the one with your photo.
Tickets or itinerary showing the date you left the U.S. and your return date
Where to find it: E-tickets, boarding passes, or your return booking.
Two identical, recent passport-style photos (2x2 inches)
Where to find it: Pharmacies and photo counters shoot them to USCIS size; pencil your name and A-Number on the back.
Receipt for the fee paid online
Where to find it: Pay on uscis.gov before your appointment and print the receipt. There is no fee waiver for this form.
Clear copies of each document, not originals
Where to find it: A copy shop, or your phone camera in good light with the whole document in the frame.

Proof of your permanent residence (or of your advance parole)
If you have it on hand: a copy of your Green Card, immigrant visa, or admission stamp
Where to find it: An old photo of your card, the visa in your passport, or your parole document (I-512).
English translation of any document in another language
Only if a document is not in English
Where to find it: Anyone fluent in both languages can translate and sign the certification with their name, date, and contact details.
