Copy of the I-94 for each person included in the application
Where to find it: Download it free at i94.cbp.dhs.gov with your passport; if it is paper, copy front and back.
USCIS fee payment (waiver only in limited cases)
Where to find it: The current amount is on the uscis.gov fee calculator. By mail you pay by card (G-1450) or bank account (G-1650); paid to USCIS separately.
Clear copies of each document, not originals
Where to find it: A copy shop, or your phone camera in good light with the whole document in the frame.

Copy of your passport page with your photo
If your category asks for it; check yours in the instructions
Where to find it: The page with your photo and details; the passport must be valid.
Letter explaining why you request more time or the change
For B-1/B-2 visitors, and other categories that ask for it
Where to find it: You write it yourself: one clear page, dated and signed.
Proof of how you will support yourself while in the U.S.
For visitors and students
Where to find it: Bank statements, proof of your income, or a letter from your supporter with their Form I-134.
Proof of the principal person's status
Only if you are a dependent (for example H-4, L-2, or F-2)
Where to find it: A copy of the I-797 (receipt or approval) for their I-129 petition, or their I-94.
