Form I-765 and the I-765WS worksheet, in the same envelope

Where to find it: Both download from uscis.gov/i-765; the I-765 carries its own photos.

Two identical, recent passport-style photos (for the I-765)

Where to find it: Pharmacies and photo counters shoot them to USCIS size; pencil your name and A-Number on the back.

USCIS fee payment

Where to find it: The current amount is on the uscis.gov fee calculator. By mail you pay by card (G-1450) or bank account (G-1650); paid to USCIS separately.

Clear copies of each document, not originals

Where to find it: A copy shop, or your phone camera in good light with the whole document in the frame.

Proof of identity

Initial request; on a renewal you do not resend what you already sent

Where to find it: Passport, birth certificate with a photo ID, your country's ID card, school ID, or driver's license.

Proof that you came to the U.S. before turning 16

Where to find it: Passport stamp, I-94, school or medical records, travel tickets, or a baptism or first-communion record.

Proof that you have lived in the U.S. since June 15, 2007

Where to find it: Rent and utility receipts, pay stubs or W-2s, school, medical, or church records, money-order receipts, children's birth certificates.

Proof that you were in the U.S. on June 15, 2012

Where to find it: Any document with your name and a date from those weeks: school, work, medical, bank, or receipts.
