Replace or renew green card

USCIS fee payment, or Form I-912 if you request a fee waiver
Where to find it: The current amount is on the uscis.gov fee calculator. By mail you pay by card (G-1450) or bank account (G-1650); paid to USCIS separately.
Clear copies of each document, not originals
Where to find it: A copy shop, or your phone camera in good light with the whole document in the frame.

Copy of your current Green Card, front and back
If you still have the card: expired, expiring, or damaged
Where to find it: The physical card; copy both sides onto one sheet if you can.
Copy of a government-issued photo ID
If your card was lost, stolen, destroyed, or never arrived
Where to find it: Passport (even expired), driver's license, work permit, or Green Card.
Copy of the latest I-797 notice for your case
Only if your card never arrived
Where to find it: The notice that came by mail for your I-485, I-751, or earlier I-90; the I-551 stamp in your passport also works.
Legal document for a name change
Only if your name changed since the card was issued
Where to find it: Marriage certificate, divorce decree, adoption decree, or court order, registered with the civil authority.
